Endoscopic Ultra Sound
Endoscopic ultrasound (EUS) is a specialized endoscopy procedure, where a flexible endoscope with an inbuilt small ultrasound device, is passed inside through the mouth into the upper part of the gastrointestinal tract (GIT). It is a non-radiation investigation. While conventional endoscopy sees only the superficial or innermost lining (mucosal surface) of the wall of the GIT, EUS offers unique additional advantage of precise and deeper visualization of organs and structures that lies beyond the luminal wall GIT e.g. pancreas, bile duct, gallbladder, adrenal, and structures located deeper parts of chest (mediastinum) and abdomen. It also helps in assessing the different layers of the wall which helps in diagnoses and staging of various tumours.

Indications
❖ Visualization and biopsy of various mediastinal diseases like lymphadenopathy (enlarged lymph nodes), lung cancer
❖ Intra-abdominal lymph node biopsy
❖ Diagnosis & biopsy of esophageal tumours & tumours within gastric wall
❖ Diagnosis & Management of CBD stones and cancers, not confirmed by other methods
❖ Diagnosis & Management of diseases of Pancreas
❖ Accurate cancer staging of oesophagus, stomach, pancreas, bile duct, ampulla.
❖ Drainage of Pseudocyst and WOPN
❖ Performing Hepatogastrostomy
❖ Celiac plexus Neurolysis for abdominal pain relief in patients with advanced pancreatic cancers.
❖ Accurate placement of glue or coils into uncontrolled internal bleeding from ruptured veins (varices).
❖ For monitoring diseases e.g. pancreatic cyst, sub-epithelial GIT lesions, and also regression of benign tumours after the intervention.
Pre-procedure preparation:
Stop taking orally 8 hours before the time of procedure. Do not take anti-diabetes medicines on the day of the procedures. If you are on blood thinning agents, please inform as they may need to be stopped/modified a few days before the procedure.

❖ EUS is a very safe procedure done under sedation that is monitored by an anaesthetist.
❖ The average ‘diagnostic EUS’ lasts about 20-30 minutes, and the patient can go home after 2-4 hours of observation.
❖ The ‘interventional EUS’ may require between 30-60 minutes to complete and is majorly performed as an inpatient procedure.
❖ Specialized endoscope which has ultrasound transducer at its tip is inserted.
❖ Suitable position of the scope is acquired for detailed evaluation of the concerned pathology.
❖ Biopsies can be taken with the help of specialized EUS needles and on site pathologist can confirm the success of the procedure if required by examining the obtained sample under microscope.
❖ Therapeutic procedures like cyst drainage, necrosectomy can be performed
