Self Expandable Metallic Stent Placement
SEMS are metallic tubes made up of Nitinol. They can be squeezed into smaller diameter and when deployed at the desired site, they can expand to the original size and shape due to their characteristic of ‘memory’. Due to this characteristic, they can be placed across the narrowed areas of esophagus, pylorus, duodenum, bile duct or colon and the compromised lumen can be expanded to relieve the obstructive symptoms. They can also be used to compress the bleeding esophageal varices when other measures to control the bleeding fail.

Method
Usually it is a day-care procedure. Some patients may require over-night hospitalization. The procedure requires sedation. The endoscope is passed through the mouth or the anus depending on the site of SEMS placement. A guidewire is passed across the narrowed area under fluoroscopic (X-ray) guidance.
SEMS is advanced either through the scope or over the guidewire till it reaches proper site. It is opened under endoscopic vision / radiological guidance and position is confirmed. It can be covered, uncovered or partially covered. The type of the stent is selected as per the indication of the SEMS placement. Uncovered stent cannot be removed afterwards as there will be tissue in-growth over the stent. Normal diet is usually resumed from the next day in most cases.
